*This giveaway is open to attendees who received a scan to enter card from a Physical Rehabilitation Network ("PRN") representative at the 2023 PPS Annual Conference. Giveaway will close on Friday, February 24, 2023 at Noon PT. All duplicate submissions cast by an existing entry user will be considered invalid. Winners will be chosen at random and will be notified via email and/or phone within 72 hours of the giveaway closing using the contact information provided in this application. PRN will assume all costs related to giveaway prize distribution. Winners may be asked to submit a photograph of themselves for use in contest promotional materials.
Tuition Giveaway winner is required to consent to the use of their likeness, name, area of residence, school name, prize amount and other contest information without limitation or further payment. Scholarship funds may be considered non-taxable under applicable federal tax laws. Please direct all questions about tax preparation to your tax advisor.
