*This giveaway is open to attendees who scan promotional displays at the Physical Rehabilitation Network ("PRN") booth at the 2022 APTA Private Practice Sections Annual Conference. Giveaway will close on November 6, 2022 at 12:00 pm CT. All duplicate submissions cast by an existing entry user will be considered invalid. Winners will be chosen at random and will be notified via email or phone and on this webpage within 72 hours of the giveaway closing using the contact information provided in this application. PRN will assume all costs related to giveaway prize distribution. Winners may be asked to submit a photograph of themselves for use in contest promotional materials.
