I can say this was the best Physical Therapy I’ve ever had! The front desk staff, my therapist Todd, and even my billing was smooth from start to finish. Don’t go anywhere but Proactive PT in Greely! They are the best!! Thank you for your help. I am back up and running again without pain.

Six weeks ago I had a total knee replacement. Having been a very satisfied past customer of Pro Active physical therapy located in Greeley I chose them to do my treatment for my knee. I am extremely pleased to say that six weeks out. I have great flexion with my knee and great extension as well. I can’t recommend Todd, Darrell, and Haley enough. I’ve worked with all three of them and They all do a great job as well as all the staff.

I have enjoyed my experience at ProActive! They have the best individuals that care about you, you're care and you're recovery. I even mentioned today as my rehabilitation come to an end, I will will miss the interactions with them all and miss getting to see them all week! I went here after ACL and meniscus surgery and I wish I would have gone elsewhere. The people at Proactive are very nice, but I feel that the care I received was not up to par. My PT only spent around 7-10 minutes with me in the hour long appointments and I had to ask to get exercises to do at home, they were never provided unless I directly asked. After the 10ish minutes (tops) with the PT, he works with a different patient or leaves the room completely. The other staff members will then cycle around and give you exercises, but it never felt like anyone was on the same page or knew where I was at in recovery. I didn’t know that straightening my leg was first priority and extremely important early until I switched to a different PT in town. Now I feel behind in my recovery because proactive didn’t set me up for success in the early weeks. At the new place, Select PT in Greeley, I had a completely different experience with 1:1 care for the entire hour, measurements of ROM (which proactive never did,) and a printed exercise regimen catered to my individual recovery. These ROM exercises are completely different than what I was doing at Proactive PT. Don’t make the same mistake I did…go get personalized care elsewhere.

My 10 year old came here after breaking his ankle and having surgery. Coming in you’re met with a very rude front desk, it’s clear they don’t want you there. Then you see a physical therapist for about two minutes. After 3.5 sessions I didn’t even know who our physical therapist was, he never introduced himself. I was later able to figure out his name is Daryl. After a brief interaction with the physical therapist, an assistant tells you an exercise to do and then leaves you to do it by yourself. They either go to help another person or to the corner of the room where the assistants hang out chatting, so it didn’t seem that them leaving you to do all of your PT alone was only because of being busy, that’s just the standard. I noticed the same with everyone there and a wide range of injuries, everyone is mainly doing their PT alone. They don’t even watch a single rep to see if you’re doing it correctly and have proper form. YouTube would be much more effective than this office for PT. Our final straw was our 4th appointment when an assistant got my son to a table and put a heat pad on his ankle and then left. We sat with the heat pad for half an hour with nobody saying a word to us. We finally just left without doing any PT. There were a couple times over the course of our sessions at ProActive that the assistants were at least friendly toward my son, we appreciated those brief moments. Otherwise, it was an absolute waste of time and money and now he’s stuck with wait times at other PT offices in the middle of his recovery, I guess it makes sense why this office was the only one easy to get into.

Commonly Asked Questions & What to Expect During Your PT Treatment

Do I need to see a doctor first?

Thanks to direct access, we can see any patient without a referral from a doctor for an initial evaluation. If our therapists believe that physical therapy is a good option for your needs, we can work with you and your physician to obtain a prescription for continued care. If your diagnosis requires additional expertise, we can also refer you to a specialist to handle your care.

What do I need to bring to my first visit?

For your first visit, be sure to bring the following items:

  • Prescription for physical therapy from your referring physician (if applicable)
  • Government issued photo ID (such as a driver’s license)
  • Insurance card(s)
  • Patient forms, which can be downloaded from this site in PDF format. Print and fill out the forms completely and bring them during your first visit.
  • Patients who are minors (under 18 years of age) must be accompanied by a parent or guardian for their first visit.

What should I wear to physical therapy?

Please wear or bring comfortable athletic/workout clothing to change into. This includes shorts or sweatpants, a t-shirt and sneakers. Your attire should also allow easy access to your injured area for treatment by your physical therapist.

What can I expect during my first visit?

During your first visit, your therapist will perform a thorough examination, in which he or she will review your physician’s recommendations, examine the problem area, ask questions about your injury or pain, develop an effective treatment plan for rehabilitation, discuss their findings with you and answer any questions you may have. Your treatment plan may include hands-on, manual therapy, a customized exercise program, stretching and physical agents such as heat, ice, ultrasound and electrical stimulation. Your physical therapist may also recommend a home program to further assist in your treatment.

How long will my appointment take?

Your first appointment will typically last from 45 minutes to an hour and a half, depending on your injury. Following appointments usually last between 30 minutes and an hour, depending on the treatment. Please arrive 15 minutes prior to the start of your appointment for check-in and paperwork. You can also download our patient forms, which can be filled out beforehand and brought with to your appointment.

How often will I need to come?

The frequency of your treatment will depend on your physician’s prescription and the plan of care developed by your physical therapist. Most patients require treatment two to three times per week. The duration of treatment will depend on your injury and personal progress. Your physical therapist will discuss this with you following your initial exam and subsequently as you progress.

What if I need to cancel an appointment?

If you are unable to attend an appointment, we require at least 24 hours' notice. However, we do understand than an occasional emergency or illness may arise, in which case you may have to cancel your appointment closer to your scheduled time.

Do you accept insurances? Will insurance cover my therapy?

We accept most major insurances including Medicare, Tricare and workers’ compensation plans. Before your initial visit, we will verify in and out of network benefits with your insurance company. The cost of therapy is determined by the benefits of your insurance plan, including the deductible and co-pay, or patient percentage. We are happy to check with your insurance company to determine the terms of your coverage. Payment for patient responsibility is due at the time of service.
