Kelly Hawkins PT Disability Parking Placard Certification

Helping Patients Access Safe and Convenient Parking

Nevada Physical and Occupational Therapists are authorized to evaluate and certify individuals who qualify for DMV disability parking placards. Our licensed providers can assess your mobility needs, complete the required medical documentation, and guide you through the application process—all in a convenient, patient-centered setting.

This service is designed to help individuals who experience difficulty walking or accessing public spaces due to medical conditions. Placards allow qualifying individuals to park in designated accessible spaces closer to entrances, improving safety and independence.

Who May Qualify for a Disability Parking Placard

Nevada law allows individuals to qualify for a disability parking placard if a licensed healthcare provider certifies that they have a condition that significantly limits mobility or function. You may qualify if you:

  • Cannot walk 200 feet without stopping to rest
  • Require assistance to walk, such as a cane, crutch, brace, wheelchair, prosthetic, or another person
  • Have a lung condition that limits breathing or endurance
  • Use portable oxygen
  • Have a cardiac condition classified as Class III or Class IV by the American Heart Association
  • Have a severe orthopedic, neurological, or arthritic condition that limits walking
  • Have a qualifying visual impairment

Both temporary and long-term conditions may be eligible, depending on severity and expected duration.
